"Company" refers to A Neuro Diagnostic Services.
"Services" refers to the medical diagnostic services provided by the Company.
"Patient" refers to the individual receiving the Services.
"Personal Data" refers to any information that can be used to identify a Patient, including but not limited to name, address, date of birth, and medical history.

2. SCOPE OF SERVICES

The Company will provide medical diagnostic services to patients, including but not limited to neurodiagnostic testing and interpretation.
The Company will use reasonable care and skill in providing the Services.

3. PATIENT RESPONSIBILITIES

Patients must provide accurate and complete Personal Data and medical history.
Patients must comply with all instructions and recommendations provided by the Company's healthcare professionals.
Patients are responsible for paying all fees and charges associated with the Services.

4. CONFIDENTIALITY AND DATA PROTECTION

The Company will maintain the confidentiality of all Personal Data and medical records.
The Company will comply with all applicable data protection laws and regulations.

5. PAYMENT TERMS

Patients must pay all fees and charges associated with the Services at the time of service.
The Company accepts cash, credit cards, bank transfers or insurance.

6. CANCELLATION AND REFUND POLICY

Patients may cancel or reschedule appointments with at least 24 hours’ notice.
Patients who cancel or reschedule appointments with less than 24 hours’ notice may be subject to a cancellation fee.

7. LIMITATION OF LIABILITY

The Company will not be liable for any damages or losses arising from the provision of the Services, except to the extent caused by the Company's gross negligence or willful misconduct.

8. INDEMNIFICATION

Patients agree to indemnify and hold harmless the Company, its officers, directors, employees, and agents from any claims, demands, or causes of action arising from the provision of the Services.

9. GOVERNING LAW

These Terms and Conditions will be governed by and construed in accordance with the laws of the United States of America.

10. CHANGES TO TERMS AND CONDITIONS

The Company may update these Terms and Conditions from time to time.
Patients will be notified of any changes to these Terms and Conditions and will be required to accept the updated Terms and Conditions before receiving further Services.

11. ENTIRE AGREEMENT

These Terms and Conditions constitute the entire agreement between the Company and Patients regarding the provision of the Services.
By using the Services, Patients acknowledge that they have read, understood, and agree to be bound by these Terms and Conditions.
